Active Membership is open to graduates of the CHIP program and Associate membership is available to any person or business that supports the principles of healthy lifestyle choices.

Graduates and spouses (or significant others) of graduates of the CHIP Program are welcome to become members of the CHIP Alumni.  Contact Sheila Snider, Executive Director at 815-394-5016 to join.  The cost of membership is $25 single or $35 for couples.  Business membership is $150.00. 

To become a member or renew your membership send an email to ssnider@rockford.edu and an application will be sent you by return email. Or better, download our brochure, fill out and return by mail. 

Dues and other donations are considered to be tax deductible.  The Chip alumni is a 501c3 tax exempt organization.  Donations are used for operating expenses and to fund educational programs to develop needed skills to maintain and enhance the health benefits derived from participation in the CHIP Program. These include classes in nutritional label reading, shopping tours, meal planning, and cooking classes.
